The BRIDGE Program helps you navigate life's challenges with one-on-one support from a Certified Community Health Worker

The BRIDGE Program connects individuals with a dedicated Community Health Worker who can help with:

  • Accessing food, housing, or health care
  • Getting back into school or work
  • Managing mental health or substance use concerns
  • Navigating transportation or legal barriers
  • Rebuilding after incarceration or major life changes


 

Our support is free and confidential. Here’s what you can expect:

  • Help with paperwork and applications (housing, Medicaid, SNAP, ID cards, etc.)
  • A caring advocate who listens and helps you create a personal action plan
  • Transportation solutions for things like dialysis or chemotherapy
  • Connections to local services that are right for your situation
  • and more!


 

The BRIDGE Program may be right for you if:

  • You’re going through a tough time and don’t know where to turn
  • You’ve been recently released from jail or prison
  • You’re living without stable housing, income, or support
  • You’re dealing with health, addiction, or mental health concerns
  • You want to make changes, but need help getting started


 

Step 1: Fill out the referral form
Just a few questions to get started. We’ll keep it private.

Step 2: Meet your Community Health Worker (CHW)
You’ll have a one-on-one conversation to set goals and start a plan.

Step 3: Get ongoing support
Your CHW will stick with you, helping you work through challenges, track progress, and connect to the right resources.
